Description
Arising distally from a segmental (tertiary) bronchus, the intrasegmental bronchus is presented from an inferior viewpoint as it courses superiorly into the lung parenchyma, its lumen narrowing as it transitions toward smaller bronchi and bronchioles. From this plane, the bronchial tube is appreciated as a branching conduit embedded deep to the visceral pleura, with adjacent parenchyma occupying the lateral and posterior field around the airway. Proximal airway caliber is greater near the segmental origin, while distal branches diverge and taper as they extend centrally within the segment. Orientation from below matters because the intrasegmental bronchus defines the internal airway map of a bronchopulmonary segment, the unit targeted in segmentectomy and in bronchoscopic localization for peripheral lesions. Surgeons and interventional pulmonologists rely on the relationship between segmental and subsegmental bronchi when planning a VATS or robotic segmental resection, and when placing dye or fiducial markers for small nodules that may not be palpable. Small airway caliber is the point. It is also where mucus plugging and inflammatory narrowing can precipitate segmental atelectasis or postobstructive pneumonia, patterns often read on CT by tracing the bronchial tree back to a culprit airway.
