Description
Airway anatomy centers on the trachea as it descends in the midline and bifurcates at the carina into the right main bronchus and left main bronchus, with emphasis placed on the right-sided pathway toward the lower lobe. Just distal to the origin of the right superior lobar bronchus, the bronchus intermedius continues inferolaterally before giving rise to the inferior (lower) lobar bronchus. Proximal-to-distal continuity is clear: mainstem bronchus, bronchus intermedius, then the lower lobar branch, with the airway lumen maintaining a gently tapering caliber and cartilaginous support. Orientation prioritizes an anterior relationship to the esophagus and posterior mediastinal structures, while remaining right of the midline. Clinically, the segment between the right main bronchus and the inferior lobar bronchus is a frequent corridor for aspirated material, given the wider, shorter, more vertical configuration of the right mainstem compared with the left.
