Description
Arising distal to the carina, the right main bronchus continues inferolaterally from the trachea and gives off the right superior lobar bronchus, after which the airway narrows into the bronchus intermedius as a short segment. Inferior to the eparterial right upper lobar bronchus, the bronchus intermedius courses toward the hilum and divides into the right middle and right inferior lobar bronchi. Cartilaginous rings, a posterior membranous wall adjacent to the esophagus, and the surrounding peribronchial connective tissue frame the lumen. Orientation on the right is steeper and more vertical than the left mainstem bronchus. Bronchus intermedius anatomy matters because it is a common choke point for right-sided obstruction and intervention. Aspirated foreign bodies, mucus plugging, and endobronchial tumors often lodge along the right mainstem and may extend into the bronchus intermedius, compromising ventilation to both the middle and lower lobes in a single lesion. During bronchoscopy, the takeoff of the right upper lobar bronchus defines the proximal limit of the bronchus intermedius, and familiarity with this segment guides selective intubation, bronchial blocker placement, and interpretation of right hilar atelectasis patterns. Small segment, big consequence.
