Centered on the male pelvic brim from an anterior perspective, the right and left common iliac veins ascend posteromedial to their companion arteries and converge just inferior to the aortic bifurcation to form the inferior vena cava. Each common iliac vein is formed by the union of the external iliac vein, coursing along the pelvic inlet toward the inguinal ligament, and the internal iliac vein draining the pelvic walls and viscera. Expect tributary patterns consistent with the iliac system, including pelvic venous channels within the hip girdle adjacent to the ilium and proximal femoral vessels continuing inferiorly into the upper thighs. Blue venous pathways contrast against thicker arterial trunks. An anterior layout like this matters when you need to teach or plan around the common iliac confluence, a frequent landmark in retroperitoneal surgery and vascular exposure. The left common iliac vein runs medial and slightly posterior to the right common iliac artery and is classically vulnerable to compression against the lumbar vertebral bodies, a setup that underlies iliac vein compression (May-Thurner) and left-sided iliofemoral deep venous thrombosis.
