Description
Anterior torso and pelvic girdle are rendered in a semi-transparent, x-ray style, with the lumbar vertebrae descending into the sacrum and coccyx at midline and the ilia flaring laterally to frame the pelvic inlet. Deep within the true pelvis, venous channels converge into the internal iliac vein on each side, receiving tributaries from the vesical and prostatic venous plexuses that sit inferior to the bladder and anterior to the rectum. Alongside these vessels, the lumbosacral plexus courses on the posterior pelvic wall, its nerve trunks emerging lateral to the sacrum and passing toward the greater sciatic foramen. Relationships matter here. Pelvic venous plexuses are clinically frustrating because they are valveless, variable, and prone to brisk, obscured bleeding when disrupted. During radical prostatectomy or cystoprostatectomy, the dorsal venous complex and periprostatic plexus can become the dominant source of blood loss, and their proximity to the pelvic autonomic nerves raises the stakes for potency- and continence-sparing dissection. This perspective also supports teaching of venous collateral pathways in iliac vein obstruction, where pelvic plexuses can enlarge and reroute flow between internal iliac, external iliac, and vertebral venous systems. Use this asset in gross anatomy and pelvic surgery teaching to orient learners to the internal iliac venous drainage pattern while keeping the spinal column and pelvic nerves as fixed landmarks. It also fits well in urology, vascular, and interventional radiology materials that discuss pelvic congestion, varices, or operative hemostasis in the male pelvis.
