Description

Anterior shoulder venous anatomy is rendered through a semi-transparent male upper torso, with the cephalic vein ascending along the anterolateral arm and entering the deltopectoral groove between deltoid and pectoralis major before piercing the clavipectoral fascia to join the axillary vein. Medial to this, the basilic system and paired brachial venules converge proximally, and the axillary trunk continues superiorly as the subclavian vein at the lateral border of the first rib, coursing deep to the clavicle toward the root of the neck. The acromion and deltoid contour frame the superficial pathways, while adjacent nerve cords of the brachial plexus track in close relationship to the axillary vessels. Layer relationships read clearly. Clinically, this anterior perspective maps the sites most often accessed and most often injured: the cephalic vein cutdown at the deltopectoral interval, ultrasound-guided axillary venous cannulation, and subclavian central venous access beneath the clavicle. Needle trajectory and catheter tip placement depend on appreciating where the subclavian vein lies anterior to the subclavian artery and inferior to the brachial plexus, and why misdirection risks pneumothorax or arterial puncture at the thoracic inlet. Venous return can also be contextualized here in thoracic outlet syndrome or upper-extremity deep venous thrombosis, where axillary-subclavian obstruction produces arm swelling and prominent collateral vessels.
