Description

Centered on the thorax, the paired lungs are rendered semi-transparently to expose the venous drainage returning oxygenated blood from the pulmonary parenchyma to the left atrium. Peripheral venules converge into segmental and lobar pulmonary veins that course centrally within the lung hilum, typically lying anterior and inferior to the corresponding pulmonary arteries, while the trachea bifurcates superiorly into the right and left main bronchi with progressive branching into bronchi and bronchioles. Red and blue vascular channels distinguish pulmonary arterial inflow from pulmonary venous outflow, and yellow nerve pathways track autonomic fibers along the bronchovascular bundles toward the pulmonary plexuses. Lobation is evident, with the right lung divided into superior, middle, and inferior lobes and the left lung into superior and inferior lobes, tapering inferiorly to rest on the diaphragm. Pulmonary veins matter because their anatomy drives both interpretation and intervention. Atrial fibrillation ablation targets ectopic foci near the pulmonary vein ostia, so appreciating the number and orientation of the veins, and their relationship to the bronchi and pulmonary arteries at the hila, helps frame risks such as pulmonary vein stenosis or atrioesophageal complications when lesions are placed posteriorly at the left atrium. This is where variants such as a common left pulmonary venous trunk or an accessory right middle pulmonary vein become more than trivia. Small structures, real consequences.
