Description
Radiating from the left atrium at the medial aspect of each lung, the superior and inferior pulmonary veins course laterally through the lung hila, returning oxygenated blood from the pulmonary venules. Semi-transparent lungs frame the central mediastinum, with the trachea descending in the midline and bifurcating at the carina into right and left main bronchi that branch into lobar and segmental bronchi. The heart sits centrally, anterior to the esophagus and inferior to the tracheal bifurcation, anchoring the venous inflow while the bronchial tree spreads peripherally toward the pleural margins. Orientation reads as an anterior, x-ray style projection. Clean anatomy. Clinically, pulmonary veins matter because they define left atrial inflow and form key landmarks for electrophysiology, pulmonary vein isolation targets the venous ostia to treat atrial fibrillation while avoiding stenosis and collateral injury to adjacent bronchi and the esophagus. Their intimate relationship to the hilar bronchi also explains why perihilar masses, lymphadenopathy, or postoperative scarring can distort venous drainage patterns and contribute to pulmonary edema or hemoptysis. For teaching, the combined airway and venous mapping clarifies why oxygenated blood returns to the left heart, a frequent point of confusion for early trainees.
