Description

Dominating the composition is a semi-transparent male heart with the left atrium positioned posteriorly and superiorly relative to the left ventricle, and with the pulmonary venous ostia clustered along its posterior wall. Right and left superior and inferior pulmonary veins course from the hila toward the midline, then converge as short trunks before entering the atrium, while smaller venules feed into these larger-caliber vessels more laterally. Aorta and pulmonary trunk are suggested anteriorly, but the emphasis stays on the posterior inflow side of the heart. Clean anatomy. High contrast. Posterior relationships matter here because pulmonary venous drainage is one of the few places where students routinely get turned around between anterior chest wall landmarks and what the surgeon or electrophysiologist encounters behind the heart. Variant pulmonary venous anatomy, such as a right middle pulmonary vein or a common left pulmonary venous trunk, changes the strategy for atrial fibrillation ablation, where lesions are placed around the pulmonary vein antra to electrically isolate triggers. The same region becomes a target in repair of total anomalous pulmonary venous return, and it is a hazard zone during left atrial surgery because the esophagus lies immediately posterior to the left atrium.
