Centered on the uterine isthmus, the narrow segment between the uterine corpus superiorly and the cervix inferiorly, the rendering would logically include the endometrial cavity tapering toward the internal os and the surrounding myometrium with its distinct junctional zone. Anterior and posterior uterine walls converge as the lumen narrows, and the transition toward the cervical canal sits inferior and slightly posterior to the usual position of the uterine fundus in anteversion. Serosal contour of the uterine body can be appreciated externally, while the endometrium and myometrium define the key internal relationships. Small region, big implications. Attention to the isthmus matters because it becomes the lower uterine segment in late pregnancy, the tissue plane targeted during low transverse cesarean delivery and a common site of cesarean scar defects (isthmocele) that can present with postmenstrual spotting, pelvic pain, or secondary infertility.
