Description

Situated between the uterine body (corpus uteri) superiorly and the cervix uteri inferiorly, the isthmus of the uterus appears as a short circumferential narrowing of the myometrium and endometrial canal. Its internal lumen continues the cavitas uteri above into the canalis cervicis below, with a subtle caliber change at the level of the internal uterine orifice (ostium internum uteri). Anteriorly the isthmus lies deep to the vesicouterine pouch and urinary bladder, while posteriorly it relates to the rectouterine pouch (pouch of Douglas). Laterally, the uterine wall at this level is approached by the uterine vessels within the broad ligament. Clinically, the isthmus is where the lower uterine segment forms in late pregnancy, and it is the usual site for a low transverse cesarean uterine incision because the myometrium is thinner and tends to bleed less than the contractile fundal region. Small differences in level matter. An incision that extends too laterally can endanger the uterine artery and ureter near the cervix, while a cut placed too inferiorly risks entering the cervix and complicating closure. The isthmus also provides an anatomic reference when discussing cervical insufficiency, internal os funneling on obstetric ultrasound, and the transition from uterine to cervical epithelium in hysterectomy specimens.
