Description
Positioned between the uterine corpus and the cervix, the uterine isthmus appears as the short, constricted segment of the uterus that tapers inferiorly toward the internal os. Superior to it lies the endometrial cavity of the body, while inferiorly the cervical canal begins, with the isthmus forming the narrow junction where the lumen changes caliber. Anterior and posterior uterine walls converge here, and the serosal contour reflects the subtle waistlike narrowing at this level. This is the segment that becomes the lower uterine segment in late pregnancy. Clinically, the isthmus matters because it is the tissue plane that distends and thins as gestation advances, defining the anatomy relevant to a low transverse (Pfannenstiel) cesarean delivery and the risk profile for uterine scar dehiscence or rupture in a trial of labor after cesarean. It is also the anatomic neighborhood of the internal cervical os, a key landmark in transvaginal ultrasound for cervical length assessment and in counseling for cervical insufficiency, where misidentifying the isthmus can lead to inconsistent measurements.
