Positioned between the uterine corpus and the cervix, the isthmus uteri appears as a short, circumferential constriction where the muscular wall narrows and the uterine cavity tapers toward the cervical canal. Superiorly it meets the lower uterine segment, while inferiorly it transitions into the supravaginal cervix and the endocervical canal. The myometrium encircles it as a continuous sleeve, with the endometrium of the uterine cavity becoming thinner as it approaches the internal uterine orifice (internal os). Lateral margins imply continuity with the broad ligament attachments and the course of the uterine vessels within the parametrial tissues, even when the render isolates the organ.
