Description
Anterior and posterior external contours of the uterus are centered on the isthmus uteri, the short segment of myometrium that lies inferior to the uterine body (corpus uteri) and superior to the cervix uteri. A shallow circumferential groove is suggested at the junction, corresponding to the subtle change in caliber where the uterine cavity narrows toward the internal os. Laterally, the isthmus blends into the uterine walls where the uterine tubes would join more superiorly at the cornua, with the cervix continuing inferiorly toward the vaginal portion. Clinically, the isthmus matters because it becomes the lower uterine segment in pregnancy, the region deliberately entered during a low transverse cesarean section to reduce bleeding and avoid the more contractile upper segment. It is also the anatomic neighborhood for cervical cerclage planning, where distinguishing cervix from isthmus affects suture placement relative to the internal os in cervical insufficiency. Small landmark. The external groove can help teach why “cervical length” measurements on transvaginal ultrasound are referenced to the internal os rather than an arbitrary external indentation.
