Description
Dorsal anatomy of the male foot is rendered with the joint capsules highlighted over the tarsus and metatarsus, spanning the talocrural (ankle) joint, talonavicular and calcaneocuboid components of the transverse tarsal joint, and the tarsometatarsal, metatarsophalangeal, and interphalangeal articulations. The capsule over the dorsal aspect appears thin and closely applied to the joint margins, with capsular reflections tracking proximally toward the distal tibia and fibula and distally along the bases and heads of the metatarsals. Beneath these superior wrappings, the dorsal surfaces of the talus, navicular, cuboid, cuneiforms, metatarsals, and phalanges define the bony contour from posterior to anterior. Clear relationships are maintained between medial and lateral columns of the foot. Dorsal capsular anatomy matters because it is where swelling, synovitis, and capsular distension become clinically apparent, and where surgical portals and incisions often pass. Arthroscopy of the ankle and dorsal approaches to the midfoot (for Lisfranc injury fixation or tarsometatarsal arthrodesis) rely on accurate expectations of how little soft-tissue thickness separates skin, extensor tendons, and capsule. Thin capsule also helps explain why dorsal osteophytes at the talonavicular or first metatarsophalangeal joint can rapidly limit dorsiflexion and produce shoe-wear pain. A tight space.
