Anteriorly, the uterine facies vesicalis (facies anterior) lies immediately posterior to the urinary bladder, separated superiorly by the vesicouterine pouch where peritoneum reflects from the uterus onto the bladder. The corpus uteri continues inferiorly into the isthmus and cervix, with the cervix positioned posterior to the bladder base and superior to the anterior vaginal fornix. Laterally, the peritoneum drapes toward the broad ligament, and the vesicouterine peritoneal reflection forms a clean landmark between uterine body and bladder dome. Clinical work on the lower uterine segment depends on this relationship. During cesarean delivery, the bladder flap is developed by incising peritoneum at the vesicouterine reflection and reflecting the bladder inferiorly to reduce the risk of cystotomy when entering the uterus. Adhesions in the vesicouterine pouch after prior cesarean section or endometriosis can tether the bladder to the anterior uterine wall and shift the usual peritoneal plane.
