Anteriorly, the uterine facies vesicalis (anterior surface of the corpus uteri) appears in profile as it relates to the urinary bladder and the intervening vesical pouch (excavatio vesicouterina). Superiorly, the fundus curves forward from the body, while inferiorly the isthmus narrows toward the cervix, with the vesicouterine reflection of peritoneum draping between uterus and bladder. The boundary between uterus and vesical pouch is emphasized, placing the bladder anterior and inferior to the uterine body in standard anatomical position. Key landmarks sit in close proximity. That peritoneal reflection matters in day-to-day pelvic surgery. During cesarean delivery, the uterovesical peritoneal fold is often incised and the bladder reflected inferiorly to protect it while entering the lower uterine segment, and adhesions at this interface after prior surgery can raise the risk of cystotomy. The same relationship helps explain why uterine enlargement (early pregnancy, leiomyomas) can change bladder capacity and contribute to urinary frequency by altering the contour of the anterior uterine wall against the vesical pouch.
