Description

Anterior uterine anatomy is rendered with emphasis on the facies vesicalis, outlining the outer boundary of the anterior uterine wall from the fundus and corpus down into the lower uterine segment as it approaches the cervix. The vesical surface lies anterior to the uterine cavity and is separated from the urinary bladder by the vesicouterine pouch (excavatio vesicouterina), with the peritoneal reflection forming a clear superior-to-inferior transition as it sweeps off the uterus onto the bladder dome. Inferiorly, the narrowing near the cervix clarifies the relationship between the isthmus uteri and the supravaginal part of the cervix, a region often described clinically as the lower uterine segment. That junction matters. During cesarean delivery, a low transverse hysterotomy is placed in the lower uterine segment to reduce blood loss and avoid the thicker, more contractile myometrium of the upper uterine body, while still respecting the close anterior relationship to the bladder and its peritoneal fold.
