Description

Anteriorly, the uterine facies vesicalis is presented as a smooth convex surface of the corpus uteri, continuous inferiorly with the narrowing isthmus uteri and then the cervix. Near the superolateral angles (cornua uteri), the round ligaments of the uterus insert and course anterolaterally toward the inguinal canal, marking the transition from the broad body to its lateral attachments. Superiorly, the fundus forms the dome-like contour above the level of the tubal ostia, while the midline remains medial to the paired ligamentous insertions. Relation to the urinary bladder is implied. Close apposition matters. This anterior surface is the operative face of the uterus during low transverse cesarean delivery and many laparoscopic exposures, where the vesicouterine peritoneal reflection and bladder flap are developed just inferior to the isthmus. Clear depiction of the isthmus helps distinguish the lower uterine segment, the site targeted for hysterotomy and also a common locus for placenta accreta spectrum when prior uterine scars distort normal planes. Round ligament orientation also guides surgeons during hysterectomy, where it is typically secured early to open the broad ligament and enter avascular spaces.
