Description
A human embryo at approximately six weeks of gestation lies curled within the amniotic cavity, its C-shaped trunk tapering to a short caudal eminence, or embryonic tail. The disproportionately large head shows the darkly pigmented optic cup of the developing eye and the surface grooves of the pharyngeal arches above the cardiac prominence, which bulges from the ventral body wall. Upper and lower limb buds appear as small paddle-like projections without differentiated digits. Ventrally, the vitelline duct and connecting stalk extend from the midgut region to the pear-shaped yolk sac, whose surface is threaded with vitelline vessels; behind them the translucent amniotic membrane, uterine wall and early placental tissue frame the scene. Week six falls within the embryonic period of organogenesis, when neural tube closure, cardiac looping and limb bud outgrowth are all under way and the conceptus is at its most vulnerable to teratogenic exposure. On transvaginal ultrasound at this stage the yolk sac and a cardiac flicker are the reassuring markers of an intrauterine pregnancy, and their absence guides the differential toward early pregnancy failure or ectopic implantation. Persistence of the vitelline duct is the origin of Meckel diverticulum, while disturbance of ventral body wall and midgut development at this time underlies omphalocele and gastroschisis.
