Description
A human embryo at approximately the fourth week of gestation lies curved within the translucent amniotic membrane, its C-shaped body dominated by the bulbous cephalic end at the upper right and tapering to a slender caudal eminence, the transient embryonic tail, at the lower left. Faint surface elevations along the dorsal contour correspond to the somite series flanking the neural tube, while shallow depressions on the head mark the developing optic and otic placodes and the branchial region. The yolk sac appears as a large orange sphere to the left, joined to the ventral embryo by the vitelline duct and connecting stalk, and the whole conceptus sits against the richly vascular endometrium lining the uterine wall. The fourth week is the core of organogenesis, when neural tube closure, heart tube looping and the appearance of limb buds all occur within a few days, making this the period of greatest teratogenic susceptibility. Failures of neurulation at this stage underlie anencephaly and spina bifida, which is why periconceptional folic acid supplementation is advised before conception rather than after a positive test. Clinically, the embryo is still too small for reliable sonographic resolution of its parts; transvaginal ultrasound at this point typically shows only a gestational sac with or without a yolk sac, and beta-hCG trends remain the main marker of viability.
