Description
Arterial inflow to the male lower leg is traced from the popliteal artery into the anterior tibial, posterior tibial, and fibular (peroneal) arteries, then distally into the dorsalis pedis on the dorsal foot and the medial and lateral plantar arteries on the plantar aspect. Along the medial ankle, the posterior tibial artery courses posterior to the medial malleolus and divides deep to the flexor retinaculum, while the anterior tibial artery passes anterior to the ankle joint to continue as dorsalis pedis between the tendons of extensor hallucis longus and extensor digitorum longus. Arterial arcades across the forefoot, including the arcuate artery and plantar arch, give rise to dorsal and plantar metatarsal and digital branches that run distally toward the toes. Venous return, rendered in blue, accompanies the arterial tree via venae comitantes and communicates with superficial channels along the dorsum and around the malleoli. For teaching, this arrangement makes the ankle and midfoot anatomic choke points. Palpation of the dorsalis pedis pulse and posterior tibial pulse depends on knowing these tendon and malleolar relationships, and both can be diminished in peripheral arterial disease or acute limb ischemia. Endovascular and surgical work also relies on these landmarks, from tibial artery revascularization to planning incisions that avoid injuring the dorsalis pedis during dorsal approaches for Lisfranc injuries or midfoot arthrodesis. Small branches matter here. They determine which angiosomes survive in diabetic foot ulceration.
