Running along the dorsum of the male forefoot, the dorsal metatarsal arteries continue distally as dorsal digital arteries that track between adjacent metatarsals and then course over the proximal and middle phalanges toward the distal phalanges. Bony landmarks include the tarsals, metatarsals, and segmented toe phalanges, with the vessels positioned superficial to the extensor apparatus and aligned longitudinally with each ray. Venous channels accompany the arterial pattern on the dorsal surface, returning proximally toward the ankle region. Color coding separates arteries from veins at a glance. Dorsal digital arterial anatomy matters any time you raise dorsal flaps or work around the web spaces. Incisions for dorsal approaches to the metatarsophalangeal and interphalangeal joints, correction of hammer toe or claw toe, and treatment of traumatic toe lacerations can place these small-caliber vessels at risk, and their close relationship to the phalanges explains why periosteal stripping or hardware placement can compromise distal perfusion.
