Running along the dorsum of the foot, the dorsalis pedis artery continues distally from the anterior tibial artery and gives rise to dorsal metatarsal arteries that course between the metatarsals toward the toes. From these trunks, dorsal digital arteries extend distally along the medial and lateral margins of each toe, sending fine transverse and longitudinal branches across the dorsal phalanges. Superficial dorsal veins accompany this arterial network in parallel but more variable paths, converging proximally toward the dorsal venous arch and then into the great and small saphenous systems. Orientation stays strictly dorsal, with the arterial caliber narrowing from proximal midfoot to distal toe tips. Appreciating this pattern matters when you need to plan dorsal approaches to the forefoot, where incisions for extensor tendon procedures, metatarsophalangeal arthrotomy, or toe deformity correction can intersect the dorsal metatarsal and digital arteries.
