Description
Arising from the renal pelvis at the renal hilum, each ureter descends inferiorly as a narrow, muscular duct continuous with the collecting system. A cutaway of the kidneys exposes the renal pyramids of the medulla, their apices forming papillae that project into minor calyces, which converge into major calyces before funneling into the pelvis. The ureters course medial to the renal parenchyma at their origin and then track caudally toward the pelvic inlet, paired bilaterally and symmetric in a typical male arrangement. That proximal ureteropelvic junction is a classic bottleneck. Congenital or acquired ureteropelvic junction obstruction produces hydronephrosis with dilation of the renal pelvis and calyces while the ureter distal to the obstruction remains relatively normal in caliber, a pattern you can teach directly off this anatomy. The renal pyramids provide the functional context for urine concentration, and the continuous lumen from papilla to pelvis explains how hematuria or infection can spread from the collecting system into the ureter.
