Description
Cut through a male kidney reveals the renal cortex as the outer rim and the renal medulla arranged into renal pyramids (pyramides renales, pyramids of Malpighi) whose apices project medially as renal papillae into the minor calyces. Minor calyces converge into major calyces that funnel inferiorly and medially into the renal pelvis, which narrows to form the proximal ureter at the ureteropelvic junction. Within each pyramid, collecting ducts and papillary ducts run from the corticomedullary junction toward the papilla, aligning the tubular flow of urine with the calyceal cup. Relationships are clear: cortex peripheral, pyramids deeper, calyces central, ureter exiting inferomedially from the hilum region. This anatomy matters when you need to localize obstruction and understand upstream dilation. A stone lodged at the ureteropelvic junction classically produces hydronephrosis with blunting and dilation of the calyces and renal pelvis, so seeing the papillae-to-calyx-to-pelvis pathway makes the mechanism intuitive. It also supports teaching reflux nephropathy and papillary necrosis, where damage at the papillary tip can disrupt drainage into a minor calyx and alter the appearance of the medullary pyramid.
