Description

Positioned retroperitoneally on either side of the abdominal aorta and inferior vena cava, the paired male kidneys are sectioned to emphasize the corticomedullary organization and the renal pyramids (pyramids of Malpighi) within the medulla. Striated medullary rays converge from the broad base of each pyramid toward an inferiorly and medially directed renal papilla, which projects into a minor calyx and then into the renal pelvis. Renal arteries enter the hilum medial to the renal parenchyma and branch toward the cortex, while renal veins drain back to the inferior vena cava; the ureters descend inferiorly from the renal pelvis as pale tubular continuations. Suprarenal (adrenal) glands cap the superior poles. The pyramid-to-calyx relationship is the anatomic basis for understanding urinary concentration and the collecting system, since collecting ducts traverse the medulla to empty at the papillary ducts into the minor calyces. Papillary tip anatomy also frames common clinical problems: obstruction by a calculus at the ureteropelvic junction can dilate the pelvis and calyces (hydronephrosis) and compress medullary tissue, and renal papillary necrosis (classically associated with analgesic nephropathy, diabetes mellitus, or sickle cell disease) targets the papilla where pyramids drain. Surgical planning relies on these internal landmarks, because endoscopic access for percutaneous nephrolithotomy aims for a posterior calyx to reach a specific papilla while limiting vascular injury to segmental branches. Orientation matters.
