Description
Framed around the abdominal aorta and inferior vena cava, the paired kidneys sit retroperitoneally with their hila facing medially, where the renal arteries enter and the renal veins drain anterior to the collecting system. Within each kidney, the renal medulla is organized into renal pyramids (pyramides renales, pyramides of Malpighi) whose bases abut the renal cortex and whose apices taper into renal papillae projecting into the minor calyces. Cream-colored ureters emerge inferiorly from the renal pelvis and descend toward the pelvis, while yellow and green strands suggest the renal plexus and regional lymphatic channels coursing along the renal vessels. Papillary anatomy matters because it is the final common pathway for urine exiting the collecting ducts, and the papillae are frequent sites of clinically recognizable injury and obstruction. Sloughed papilla in renal papillary necrosis (classically associated with diabetes mellitus, analgesic nephropathy, and sickle cell disease) can obstruct a minor calyx or ureter, producing colicky pain and hematuria, while medullary nephrocalcinosis and papillary tip calcifications help explain recurrent stones. The vascular emphasis also supports teaching of segmental renal arteries as end arteries, a practical point during partial nephrectomy or renal trauma when devascularized wedges map to arterial territories rather than to the pyramids themselves. Use this artwork in gross anatomy and renal physiology teaching to connect medullary architecture to urine concentration, and in urology or radiology materials discussing calyceal anatomy, papillary necrosis, or stone disease patterns at the papillae.
