Description

Shown in three-dimensional gross anatomy, the paired kidneys are sectioned to expose the renal cortex peripherally and the renal medulla centrally, where the renal pyramids (pyramides renales, pyramids of Malpighi) taper toward the renal papillae. Each papilla projects into a minor calyx, and the minor calyces converge into major calyces that funnel medially into the renal pelvis. From the pelvis, the ureter begins at the ureteropelvic junction and descends inferiorly as a tubular conduit for urine. Corticomedullary contrast is clear. Attention to the papilla-to-collecting system transition matters because it is where the terminal collecting ducts (ductus papillaris, ducts of Bellini) empty and where urine first becomes a free-flowing column, susceptible to obstruction, infection, and bleeding. This is the anatomic substrate for medullary nephrocalcinosis and papillary necrosis (classically associated with analgesic nephropathy, diabetes mellitus, and sickle cell disease), and it also clarifies why upper tract obstruction at the ureteropelvic junction produces calyceal and pelvic dilation before ureteral dilation. Stones often lodge here.
