Description

Paired kidneys sit superiorly with their renal pelves funneling medially into the proximal ureters, each tubular duct descending inferiorly toward the pelvis as a narrow, smooth-walled conduit for urine. The stylized renal substance suggests pyramids radiating toward a central collecting region, with the ureter arising at the ureteropelvic junction and continuing as a slender column. Left and right ureters run bilaterally, lateral to the great vessels in the retroperitoneum before turning anteromedially toward the urinary bladder. Lumen caliber is the visual theme. That narrow lumen is not uniform, and the common physiologic constrictions matter clinically: at the ureteropelvic junction, where the ureter crosses the iliac vessels at the pelvic brim, and at the ureterovesical junction. These are the typical arrest points for ureteric calculi, correlating with colicky flank pain, hydronephrosis proximal to the obstruction, and hematuria. In the male pelvis, the distal ureter passes posterior to the ductus deferens near the bladder, a relationship surgeons protect during pelvic and inguinal procedures.
