Description
Arising from the gallbladder wall near the neck and body, the cystic vein (venae cysticae) courses superiorly toward the hepatic surface, draining into adjacent intrahepatic portal venous branches rather than the systemic caval system. From an inferior perspective, the fundus of the gallbladder lies most anterior and inferior, while the cystic duct runs posteromedially to join the common hepatic duct and form the common bile duct. The cystic artery typically approaches the gallbladder in the hepatocystic triangle, with venous tributaries tracking back from the peritoneal surface toward the porta hepatis. Small venules in the subserosal plane converge rapidly. Short, direct, and easy to miss. Venous drainage of the gallbladder matters because it explains both operative bleeding patterns and routes of inflammatory spread. During laparoscopic cholecystectomy, traction on the infundibulum and dissection in Calot’s triangle can avulse small cystic venules and cause persistent oozing that is harder to control than an arterial bleed, especially when the cystic vein drains directly into liver parenchyma. This relationship also helps when teaching why acute cholecystitis can produce adjacent hepatic hyperemia and why portal venous seeding can occur in biliary infection.
