Description
Angled laterally, the gallbladder is rendered with its fundus inferior and its neck directed superiorly toward the biliary confluence, where the cystic duct courses medially to join the common hepatic duct and form the common bile duct. A blue cystic vein (often a pair of small venules rather than a single trunk) tracks along the neck and adjacent body of the gallbladder, running superficial to the fibromuscular wall and paralleling the cystic artery shown in red. Proximally, the venous channel is oriented toward the hepatocystic triangle (Calot triangle) region, the same confined space where the cystic duct, cystic artery, and variable accessory ducts intersect. Surgeons care about the cystic vein because it is a frequent source of obscuring bleeding during laparoscopic cholecystectomy, and it can tether the gallbladder neck to the liver bed when inflammation thickens the peritoneal reflections. This lateral perspective also reinforces a practical point: cystic venous drainage commonly empties directly into the hepatic sinusoids via small veins in the gallbladder fossa, so venous anatomy does not mirror the more familiar arterial pattern and can be more variable than trainees expect. Small structure, common nuisance.
