Layered over the anterior thoracic wall, the female mammary gland sits superficial to the pectoralis major fascia, with lobules embedded in subcutaneous adipose tissue and converging lactiferous ducts that course anteriorly to the nipple and areola. Medially, parenchyma approaches the parasternal region, while laterally it extends toward the axillary tail of Spence along the anterior axillary fold. Arterial supply is suggested by branches consistent with the internal thoracic (medial) and lateral thoracic systems (lateral), accompanied by superficial and deep venous channels. Cutaneous innervation tracks with the anterior and lateral cutaneous branches of intercostal nerves, running in a segmental pattern through the breast toward the nipple-areolar complex. For surgical anatomy, this relationship to the pectoral fascia is the plane exploited in mastectomy and in subpectoral implant placement, and it also explains why pectoralis major contraction can accentuate tethering when tumor involves Cooper’s suspensory ligaments. Lymphatic drainage matters just as much. Flow from subareolar and peripheral plexuses toward axillary nodes underpins sentinel lymph node biopsy and the patterns of metastasis in breast carcinoma, while medial channels account for internal mammary node involvement that can be missed on limited axillary assessment.
