Description

Anterior thoracic wall anatomy is presented with the female mammary gland in situ, layered superficial to the pectoralis major and the rib cage. Lobules of glandular tissue sit within adipose stroma, converging toward the nipple and areola region as lactiferous ducts, while suspensory (Cooper) ligaments tether the skin to the deeper fascia. Red arterial channels and blue venous or lymphatic pathways course through the parenchyma toward the axilla and parasternal region, and yellow cutaneous nerves approach the breast from the lateral and anterior cutaneous branches of the intercostal nerves. Spatially, the bulk of glandular tissue lies anterior to the pectoral fascia, with neurovascular structures running deep to superficial as they arborize toward the skin. This view matters because the breast is not an isolated organ, it is a modified sweat gland complex draped over the pectoral musculature, and that relationship guides both imaging interpretation and operative planes. Patterns of lymphatic drainage toward axillary nodes (and medially toward parasternal nodes) underpin staging and sentinel lymph node mapping in breast carcinoma. The neurovascular overlay also frames practical problems, from postoperative nipple-areolar complex sensibility to iatrogenic bleeding along perforators from the internal thoracic and lateral thoracic systems. Small structures, big consequences.
