Description
Centered on the shoulder girdle, the paired deltoid muscles cap the lateral aspect of the proximal humerus, their fibers converging to a V shaped insertion on the deltoid tuberosity. Anterior (clavicular) fibers arise from the lateral third of the clavicle, lateral (acromial) fibers from the acromion, and posterior (spinal or scapular) fibers from the spine of the scapula, wrapping from anterolateral to posterolateral around the glenohumeral region. Beneath the semi transparent soft tissues, the clavicles sit superior to the rib cage, the scapulae lie posterolaterally on the thoracic wall, and the humeri descend laterally with the radius and ulna continuing distally in anatomical position. Bony landmarks stay readable. Functionally, this depiction supports teaching of compartmental fiber orientation and how it maps to motion, anterior deltoid biasing flexion and internal rotation, middle deltoid driving abduction, and posterior deltoid contributing to extension and external rotation. Palpation and injection anatomy depend on the same surface topography, with the safe intramuscular injection zone typically placed in the thickest central portion of the muscle, several centimeters inferior to the acromion, to reduce risk to the axillary nerve and posterior circumflex humeral artery as they course deep to the deltoid near the surgical neck. For surgeons, the deltopectoral interval (between deltoid and pectoralis major) and deltoid splitting approaches both hinge on respecting these attachments and neurovascular relationships.
