Description
Presented in lateral profile on the fibular side, the knee is organized around the distal femur articulating with the proximal tibia, with the lateral femoral condyle positioned superior to the lateral tibial plateau. Anteriorly, the patella sits within the trochlear groove and relates distally to the tibial tuberosity via the patellar ligament, while hyaline articular cartilage caps the opposing bony surfaces. Along the lateral joint line, the fibular (lateral) collateral ligament spans from the lateral femoral epicondyle to the head of the fibula, lying superficial to the capsule and posterior to the patellar tendon. Alignment is clear. Lateral knee anatomy matters because stability here differs from the medial side: the fibular collateral ligament remains distinct from the lateral meniscus, and injury patterns follow that separation. Varus stress classically tests this ligament, and combined trauma can involve the posterolateral corner, including the popliteus tendon and arcuate complex, producing rotational instability that is easy to miss on a standard anterior view. For surgical planning, the fibular head also serves as a landmark where the common fibular (peroneal) nerve is at risk during approaches to the lateral compartment or proximal tibia.
