Description
Medially, the distal femur and proximal tibia meet at the tibiofemoral articulation, with the medial femoral condyle positioned superior to the medial tibial condyle and separated by articular cartilage. Spanning the medial joint line, the tibial collateral ligament (medial collateral ligament) runs from the medial epicondyle of the femur to the medial surface of the tibia, broad and fan-shaped compared with the cordlike lateral collateral ligament. The patella sits anterior to the distal femur as part of the patellofemoral joint, even though a true medial view keeps the trochlear contact area only partly in profile. Medial meniscal fibrocartilage is typically suggested deep to the MCL at the medial tibial plateau. A clean ligament silhouette. Clinically, this perspective centers the structures injured in valgus stress. The superficial MCL is the primary restraint to valgus angulation at 20 to 30 degrees of knee flexion, and its broad distal attachment explains why tenderness often tracks several centimeters below the joint line after a grade I or II sprain. Close apposition of the deep MCL and medial capsule to the medial meniscus underlies the classic teaching point that meniscal tears can accompany MCL trauma, even though the old “unhappy triad” is more complex in modern series. Orthopaedic exam maneuvers such as the valgus stress test and decisions about bracing versus operative repair become easier to explain when the ligament’s femoral and tibial footprints are shown against the medial condyles.
