Description
Seen from the posterior aspect of an adult male knee (genu), the medial and lateral femoral condyles frame the intercondylar fossa superiorly while the corresponding tibial condyles and tibial plateau sit inferiorly, separated by the wedge profiles of the medial and lateral menisci. The posterior horn of each meniscus lies closer to the viewer, and the posterior cruciate ligament (PCL) can be appreciated coursing from the posterior intercondylar area of the tibia proximally, anteriorly, and medially toward the lateral surface of the medial femoral condyle. Posterior capsular structures occupy the interval between femur and tibia, with the popliteal surface of the distal femur forming the floor of the popliteal region. Clear joint geometry. Clinical teaching benefits from a posterior perspective because it aligns with how many injuries declare themselves on imaging and at arthroscopy. PCL tears, tibial avulsion fractures at the PCL insertion, and posterior meniscal root pathology are easier to conceptualize when you can track how the ligament and posterior horns sit relative to the intercondylar notch and tibial plateau. This relationship also underpins posterior sag testing and guides tunnel placement during PCL reconstruction, where a few millimeters of malposition can shift graft isometry and alter posterior tibial translation.
