Description
Oriented from an inferior perspective, the third lumbar vertebra (L3) is presented with the vertebral body positioned anteriorly and the vertebral arch forming a posterior ring around the vertebral foramen. Inferior articular processes project caudally from the junction of the laminae and pedicles, and their inferior articular facets face anterolaterally to meet the superior articular facets of L4. Posteriorly, the spinous process extends in the midline, while transverse processes (costal processes in the lumbar region) reach laterally from the pedicle region. Clear facet geometry. Seeing L3 from below is the cleanest way to teach lumbar zygapophysial (facet) joint orientation and how it governs motion, favoring flexion and extension while resisting axial rotation compared with the thoracic spine. That relationship becomes clinically concrete in facet arthropathy and degenerative spondylolisthesis, where altered loading and capsular laxity can produce mechanical low back pain and segmental instability, often localized with facet blocks and treated with medial branch radiofrequency ablation. The inferior view also supports procedural planning by making the articular processes and adjacent lamina easier to conceptualize when correlating with oblique fluoroscopic projections. Use this plate in gross anatomy and musculoskeletal anatomy courses to contrast lumbar versus cervical facet planes, and in radiology or pain medicine teaching files to explain target selection for lumbar facet injections at L3 to L4.
