Description
Emerging from the superior lobe of the right lung, the right superior pulmonary vein courses medially through the pulmonary hilum and enters the left atrium on its right posterolateral aspect. At the hilum it lies anterior and inferior to the right pulmonary artery, and it sits anterior to the main bronchus, relationships that are easier to teach when the bronchial tree and pulmonary vessels are rendered together. Tributaries from the apical, posterior, and anterior bronchopulmonary segments typically converge into this vein, while the trachea and right main bronchus remain superior and posterior as they descend toward the carina and lobar bronchi. Coronary vessels on the cardiac surface provide orientation, with the venous return shown in blue and oxygenated pulmonary venous flow in red. For cardiothoracic practice, the right superior pulmonary vein is a key landmark during right upper lobectomy and hilar dissection, where the surgeon must distinguish it from the right pulmonary artery and avoid avulsion at its thin-walled atrial junction. Electrophysiology teams also target pulmonary venous ostia during atrial fibrillation ablation, and the right superior vein often defines the superior portion of the lesion set at the left atrium. Small anatomic variants matter: a separate right middle lobe pulmonary vein may drain independently to the left atrium or join the right superior pulmonary vein, a common source of confusion on CT angiography and in the operating room.
