Description
Posteriorly, the second thoracic vertebra (T2) is isolated from the surrounding vertebral column, with the spinous process projecting inferiorly and overlapping the laminae in the typical thoracic pattern. Paired laminae meet in the midline to form the vertebral arch, flanking the vertebral foramen, while the superior and inferior articular processes sit posterolaterally at the pedicle lamina junction. Laterally, the transverse processes extend to either side and carry transverse costal facets for articulation with the tubercle of the second rib. Facet joints frame the posterior elements. This posterior view is the one you want when teaching surface anatomy and procedural corridors in the upper thoracic spine, because the long spinous processes and steep laminar angles dictate midline access. T2 also marks a transition zone where cervical mobility gives way to the rib-stabilized thoracic segment, so fractures and ligamentous injuries here can behave differently than at mid-thoracic levels. Pedicle screw placement at T2 demands respect for thoracic pedicle morphology and the proximity of the spinal canal, and the costovertebral articulations help orient level identification during posterior approaches.
