Description
Viewed through a cross-section of the adult male skull, the calvaria is opened to expose the convexity of the cerebral hemispheres with prominent gyri and sulci. Superior cerebral veins course superficially within the subarachnoid space, running from the cortical surface superiorly and medially toward the superior sagittal sinus along the midline. Small bridging veins traverse the subdural space to reach the dural venous sinus, creating a clear relationship between cortex, dura mater, and inner table of the skull. Portions of the orbit and nasal cavity anchor the cranial orientation anteriorly. Venous drainage of the cerebral convexity is where anatomy becomes clinical fast. Stretching or tearing of bridging veins between the superior cerebral veins and the superior sagittal sinus underlies many acute and chronic subdural hematomas, classically after acceleration-deceleration injury or in older patients with cerebral atrophy. Neurosurgeons also work directly along this corridor during parasagittal craniotomy and superior sagittal sinus exposure, where preserving dominant cortical draining veins can prevent venous infarction and postoperative edema.
