Description
Beginning at the renal pelvis within each kidney, the paired ureters descend retroperitoneally along the posterior abdominal wall, running anterior to psoas major and crossing the pelvic brim near the bifurcation of the common iliac vessels. As the sequence advances into the female pelvis, each ureter courses inferiorly and anteromedially on the lateral pelvic wall toward the urinary bladder, with the distal segment passing close to the uterine artery and the cervix before entering the bladder at the ureterovesical junction. Peristaltic waves travel distally along the muscular wall, reinforcing the ureter’s role as an active conduit rather than a passive tube. Small details matter. Clinically, the animation maps several classic narrowing points where ureteral calculi lodge: the ureteropelvic junction, the crossing at the iliac vessels, and the intramural ureter as it tunnels obliquely through the bladder wall to support the anti-reflux mechanism. That distal pelvic relationship to the uterine artery is the setup for iatrogenic injury during hysterectomy or uterine artery ligation, where the ureter can be kinked, ligated, or transected if its anteromedial turn toward the bladder is not respected. Seeing the ureter’s changing depth and direction over time makes those spatial relationships easier to teach than a single diagram, and the motion cueing of urine propulsion clarifies why proximal dilation develops when an obstruction is fixed distally.
