Description

Beginning in the renal pelvis at the hilum of each kidney, the paired ureters descend retroperitoneally along the anterior surface of psoas major, lateral to the great vessels and medial to the ascending and descending colon. The animation tracks their course inferiorly, showing the gentle medial drift as each ureter crosses the pelvic brim anterior to the common iliac bifurcation (or external iliac artery) and then arcs anteromedially within the pelvis toward the bladder. Segment by segment, the wall layers are clarified, including urothelium, lamina propria, and the muscularis with longitudinal and circular smooth muscle that drives peristaltic urine transport. At the vesicoureteric junction, the ureter is shown passing obliquely through the detrusor to open at the superolateral angles of the trigone. A key pelvic relationship appears as the distal ureter runs inferior to the uterine artery near the cervix, the classic water under the bridge crossing. Ureteral anatomy matters most when things go wrong. Calculi commonly lodge at the ureteropelvic junction, at the pelvic brim crossing, or at the ureterovesical junction, and the animated progression of peristaltic waves helps explain intermittent colic, upstream hydroureter, and hydronephrosis. In gynecologic surgery, the distal ureter is vulnerable during hysterectomy, uterine artery ligation, and dissection of the cardinal ligament, and the sequential spatial cues in this animation make that risk easier to teach than a single still frame.
