Description

Beginning at the renal pelvis, each ureter descends retroperitoneally along the anterior surface of psoas major, then crosses the pelvic brim near the bifurcation of the common iliac vessels before entering the true pelvis and turning anteromedially toward the urinary bladder. The animation tracks the ureter’s variable caliber, highlighting the typical physiologic narrowings at the ureteropelvic junction, at the iliac vessel crossing, and at the ureterovesical junction as the intramural segment courses obliquely through the bladder wall. Within the female pelvis, the ureter runs inferior to the uterine artery near the cervix (water under the bridge), passes adjacent to the lateral fornix of the vagina, and approaches the posterolateral bladder base where it opens at the trigone. Peristalsis is visible. These relationships matter most where ureters are injured or obstructed. Ureteric calculi lodge at the narrowings and generate predictable pain patterns, from flank pain near the kidney to groin pain as the stone descends, and the sequential depiction makes the propagation of ureteral peristalsis and upstream distension easier to grasp than a single frame. Gynecologic surgery is the other pressure point: during hysterectomy, salpingectomy, or uterine artery ligation, the distal ureter’s proximity to the uterine artery and cardinal ligament explains why thermal injury, kinking, or transection can occur, and why postoperative urinary leakage or hydronephrosis may follow.
