Description
Framed behind the manubrium and upper body of the sternum, the female thymus appears in the anterior superior mediastinum as paired right and left lobes separated by a midline cleft and invested by a thin capsule with septa extending inward. As the camera advances and subtly rotates, the lobes are seen draping anterior to the pericardium and great vessels, with their inferior poles tapering toward the cardiophrenic angles while the superior poles extend toward the lower neck. Relationships to the left brachiocephalic vein, superior vena cava, aortic arch and its branches, and the anterior surface of the trachea become clearer as depth cues and sequential reveals define what lies anterior, posterior, and lateral. Thymic position matters any time an anterior mediastinal mass is considered on chest radiography or CT, where thymic hyperplasia can mimic neoplasm and normal residual thymic tissue can be mistaken for lymphadenopathy. Motion adds clarity: a stepwise peel-back of surrounding planes makes it easier to distinguish thymic contours from adjacent mediastinal fat, and to appreciate how a thymoma can encroach on the brachiocephalic veins or abut the pericardium without originating from it. Surgical relevance follows naturally, since extended thymectomy for myasthenia gravis depends on recognizing lobar extent and the cervical “tails” that can hide superiorly near the thyroid and inferiorly along the pericardiophrenic region.
