Description
Arising from the dorsal venous network of the foot, the medial marginal vein courses along the medial border of the dorsum, tracking distally near the hallux and proximally along the first metatarsal toward the medial malleolus. Superficial tributaries from the dorsal metatarsal veins and dorsal digital veins converge into this channel, which remains subcutaneous throughout its course. Proximally, the vessel aligns with the anterior aspect of the medial ankle where it continues into the great saphenous vein. Palpation and cannulation at the dorsomedial foot can be deceptively straightforward, and this view clarifies why the medial marginal vein is often selected when other superficial sites are limited, yet still prone to rolling and infiltration because it overlies mobile soft tissue rather than firm fascia. Its continuity with the great saphenous system also explains the clinical pathway for superficial thrombophlebitis extending from the foot toward the saphenofemoral junction, and why varicosities may cluster along the medial leg. Understanding the vessel’s relationship to the first ray and medial malleolus helps avoid confusing it with more central dorsal venous channels when mapping reflux patterns or planning access.
