Description

Running from the popliteal fossa into the anterior compartment, the anterior tibial artery passes through the proximal opening in the interosseous membrane and then descends on the membrane between tibialis anterior (medial) and extensor digitorum longus (lateral), accompanied by venae comitantes. Distally, the vessel crosses the anterior aspect of the ankle joint and continues as the dorsalis pedis artery over the talus and navicular, with its course framed medially by the tibial shaft and laterally by the fibula. Bony landmarks from the distal femur through the tibia and fibula to the tarsals, metatarsals, and phalanges orient the arterial path in standard anatomical position. Clean relationships. Anterior tibial artery anatomy matters when you need to localize the source of ischemic symptoms on the dorsum of the foot or interpret an absent dorsalis pedis pulse without guessing whether the problem lies proximally at the popliteal level or within the anterior compartment itself. This anterior projection also clarifies the artery’s vulnerability in anterior compartment syndrome, where rising compartment pressure compromises perfusion long before skin changes appear. For reconstructive planning, the view supports discussion of the anterior tibial and dorsalis pedis systems as donor and recipient vessels in distal lower-limb free flaps and pedal bypass targets.
