Presented from an anterior aspect, the ninth thoracic vertebra (T9) is centered on the vertebral body with the pedicles forming the short, stout bridges to the vertebral arch on either side. Near the superolateral margins of the body, the superior costal demifacets sit just lateral to the posterior edge of the vertebral body and close to the pedicle junction, marking the articular surface for the head of the ninth rib. Inferiorly, the contour transitions toward the inferior vertebral endplate, while the transverse processes and spinous process remain largely posterior and are not the primary emphasis in this projection. Landmarks are clean. T9 matters because it sits at the mid-thoracic level where rib articulation and segmental stability constrain motion, so small changes in bony anatomy can translate into pain patterns and procedural difficulty. The costovertebral joints at this level are implicated in thoracic wall pain syndromes and can mimic visceral pathology, and the demifacet location guides image-guided costovertebral or paravertebral injections by helping you anticipate where the rib head relates to the vertebral body on anterior-posterior imaging. This is also a practical reference point for counting thoracic levels in surgery or radiology when the scapular landmarks are unreliable.
