Description
Arcuate superiorly, the uterine fundus forms the dome of the uterus above the level of the tubal ostia, continuous inferiorly with the uterine body and tapering toward the isthmus. The myometrium dominates the wall thickness, with a thin outer perimetrium and an inner endometrium lining the cavity; laterally, the cornua mark where the uterine tubes would enter. An anterior surface relates to the vesicouterine reflection and bladder, while the posterior surface faces the rectouterine pouch (Douglas). Thick muscle. Clear contours. For teaching uterine anatomy, the fundus is the landmark clinicians use to orient the uterus on bimanual exam and on transabdominal ultrasound when estimating gestational age by fundal height. Its contractile myometrium is the engine of postpartum involution and a primary target in uterine atony, the commonest cause of postpartum hemorrhage, where tone at the fundus is assessed immediately after delivery. The same region often hosts intramural or subserosal leiomyomas that distort the uterine contour and can complicate myomectomy planning by altering serosal planes near the cornua.
